Page - 1, l - 1; dīkṣākramavidhiḥ॥ hariḥ oṃ॥ kriyā dīkṣāvidhirlikhyate॥ atha dīkṣāṃ pravakṣyāmi devi saṃkṣepata śṛṇu। vedābhyāsaika nirataṃ devī bhakti samanvitam॥ evaṃ prakāraṃ taṃ śiṣyaṃ pragṛhedgurupuṅgavaḥ।

Page - 8, l - 1; śrīpātre tu kuleśāni nikṣipettarpayetkramāt। śiṣyamāhūya dīkṣārthaṃ yathāvatkriyatāṃ guruḥ॥ balyudvāsanakāle tu ucchiṣṭaṃ devi dīyatām। iti parānandatantre sapādalakṣagranthe madhyakhaṇḍe dvitīyapāde dvātriṃśaddikṣā saṃpuṭitāmnāya dīkṣākramo nāma pañcamodrekaḥ। hariḥ oṃ । gurubhyo namaḥ।
