This mantra seems to be a rare one. This contains both the bījākṣara-s of śakti (śrīvidyā) and śiva. In the margin of fol. 93a it is written as "makuṭāgame saṃmelanamantram". But in the printed text of the said āgama and in the IFI.T.19 it is not found

ṣaṇmukhañca gaṇādhīśaṃ sāmbañca parameśvaram। mama duḥkhavināśāya santataṃ cintayāmyaham॥ asya śrīmanvādi(?) cidambaranaṭanasaṃmelanamahāmantrasya sadāśivaṛṣaye namaḥ। devīgāyatrīchandase namaḥ।
